In early March 2023, an incident occurred related to a large unsuccessful transaction in decentralized finance (DeFi). Aave and CoW Swap protocols, two leading players in the DeFi market, have published detailed reports on the causes and consequences of this event.

According to the CoW Swap report, a $50 million transaction was initially submitted through a private RPC (Remote Procedure Call), but somehow ended up in the public mempool (a pool of unconfirmed transactions). This allowed other traders to intercept and execute it earlier, leading to significant losses for CoW Swap users.

In turn, Aave published its own investigation, in which it claims that its side is not responsible for what happened. In their opinion, the problem was in the incorrect configuration of the CoW Swap price oracle, which led to such an unfavorable outcome of the transaction.

This incident clearly demonstrates how vulnerable even the most reliable DeFi protocols can be in the face of technical glitches and errors in settings. Despite all the security measures, the human factor and flaws in algorithms can still lead to serious financial losses for users.
